29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the employer's energy control procedures required by this standard: (a)Smithfield Packaged Meats Corp. at 1400 North Weber Avenue, Sioux Falls, SD 57103: On or about an at times prior to February 21, 2025, the employer did not ensure specific energy control procedures to deenergize the Hot Dog Peeler Equipment within the Hot Dog Production Area were utilized prior to operators accessing the machine area/danger zone. This condition exposed employees to crushing, amputation, shock, and air embolisms while accessing the equipment.
