Employees feeding wood crates into the machine were exposed to an in-running nip point hazard regarding the pneumatically driven chains and sprockets used to stabilize and move the material through the notcher's point of operation. This condition exposed employees to amputation hazards. (b)Mueller Pallets, L.L.C. at 27059 Mueller Place, Sioux Falls, SD 57108: On or about and at times prior to February 25, 2025, the employer did not ensure employees were protected from injury in that, the exposed chain and sprocket on Section 2 of the Pallet Stacker (Serial Number: A432218) was not effectively guarded. (b)Mueller Pallets, L.L.C. at 27059 Mueller Place, Sioux Falls, SD 57108: On or about and at times prior to February 25, 2025, the employer did not ensure the electric outlet located near the Stacker in the East Building Cutting Room, was covered and free from openings. This condition exposed employees to shock and burn hazards. (c)Mueller Pallets, L.L.C. at 27059 Mueller Place, Sioux Falls, SD 57108: On or about and at times prior to February 25, 2025, the employer failed to secure electric cords within the electric outlet /panel for the Vertical Saw in the East Building Cutting Room, in that extension cords are used with the equipment plugged directly into an open wall socket, approximately 100 inches away from the wall. This condition exposed employees to shock and burn hazards.

(3) The location and availability of the written hazard communication program, including the required list(s) of hazardous chemicals, and safety data sheets required by this section.
