29 CFR  1910.95(d)(1):When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:  The Pullman Company, LLC DBA Tenneco Automotive located at 11800 County Road 425 in Napoleon, Ohio 43545:  On or about February 25, 2025 the employer did not develop and implement an noise monitoring program when employees' exposures to noise may equal or exceed an exceed an 8-hour time-weighted average of 85 decibels.   Instance A  On April 15, 2025 an employee operating welding cells 8161 and 8160 was exposed to noise at 172.5% of the permissible daily dose, or an average sound level of 94.3 dBA, as measured over 452 minutes of sampling. This dose is equivalent to an 8-hour TWA exposure 93.9 dBA accounting for the period of 28 minutes unmonitored.  Instance B   On April 15, 2025 an employee operating welding cell 8162 was exposed to noise at 192.7% of the permissible daily dose, or an average sound level of 95.2 dBA, as measured over 448 minutes of sampling. This dose is equivalent to an 8-hour TWA exposure 94.7 dBA accounting for the period of 32 minutes unmonitored.

29 CFR  1910.124(g)(2): Employees who  worked with liquids that could burn, irritate, or otherwise harm their skin were not provided with an emergency shower and eye-wash station close to the dipping and coating operation:   The Pullman Company, LLC DBA Tenneco Automotive located at 11800 County Road 425 in Napoleon, Ohio 43545:  Instance A  On or about February 26, 2025 the employer did not ensure suitable facilities for quick drenching or flushing of the eyes and body were available within the work area when employees were exposed to injurious and corrosive chemicals. Employees maintaining the Udylite Line were exposed to hazards associated with corrosives in that the eye flushing station when actuated ran brown water before running clear water.   Instance B  On or about February 26, 2025 the employer did not ensure suitable facilities for quick drenching or flushing of the eyes and body were available within in the work area when employees were exposed to injurious and corrosive chemicals. Employees maintaining the Rotary Lines were exposed to hazards associated with corrosives in that a quick drenching facility was not available in the area for immediate use.

29 CFR  1910.138(b):The employer did not base selection of appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the tasks to be performed, conditions present, duration of use, and the hazards and potential hazards identified:  The Pullman Company, LLC DBA Tenneco Automotive located at 11800 County Road 425 in Napoleon, Ohio 43545:  On or about February 26, 2025, the employer did not ensure employees handling parts coated with metal working fluids and refilling machine coolant at Press 7795 were provided with appropriate hand protection. Employees were exposed to hazards associated with 3-amino-4-octanol and hexahydro-1,3,5-tris (3methoxypropyl)-1,3,5-triazine, and base oil such as, but not limited to skin irritation, redness, dryness and cracking in that the provided gloves did not provide protection from chemical liquids.

29 CFR  1910.146(d)(5)(i):Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not evaluate permit space conditions when entry operations were conducted by testing conditions in the permit space to determine acceptable entry conditions:  The Pullman Company, LLC DBA Tenneco Automotive located at 11800 County Road 425 in Napoleon, Ohio 43545:  On or about February 11, 2025 the employer did not ensure the conditions in permit spaces were tested to determine if acceptable entry conditions exist before entry is authorized to begin. Employees were exposed to atmospheric hazards associated with entries into permit spaces such as, but not limited to, Udylite Tanks, Paint Line tanks, Draw Press Pits in that the conditions in the spaces were not tested for oxygen, combustible gases and vapors, and toxic gases and vapors.
