Instance A  On April 15, 2025 an employee operating welding cells 8161 and 8160 was exposed to noise at 172.5% of the permissible daily dose, or an average sound level of 94.3 dBA, as measured over 452 minutes of sampling. This dose is equivalent to an 8-hour TWA exposure 93.9 dBA accounting for the period of 28 minutes unmonitored.  Instance B   On April 15, 2025 an employee operating welding cell 8162 was exposed to noise at 192.7% of the permissible daily dose, or an average sound level of 95.2 dBA, as measured over 448 minutes of sampling. This dose is equivalent to an 8-hour TWA exposure 94.7 dBA accounting for the period of 32 minutes unmonitored.

Employees maintaining the Udylite Line were exposed to hazards associated with corrosives in that the eye flushing station when actuated ran brown water before running clear water.   Instance B  On or about February 26, 2025 the employer did not ensure suitable facilities for quick drenching or flushing of the eyes and body were available within in the work area when employees were exposed to injurious and corrosive chemicals. Employees maintaining the Rotary Lines were exposed to hazards associated with corrosives in that a quick drenching facility was not available in the area for immediate use.
