On or about February 28, 2025, in the facility, employees were exposed to electrical shock hazards due to forklift damaging left side of the Ametek Prestolite Power Model #880H3-12G Forklift Battery Charger; moreover, facility not having fixed barriers preventing forklifts from striking battery charging systems.

On or about February 28, 2025, in the facility, the Nissan electric forklift truck, Model MCT1B2L20S, was not kept in safe working condition as the load backrest was not secured with the required bolts, thereby exposing employees to potential struck-by hazards from fallen product.
