29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) On or about February 17, 2025, at the corn-chip department, the hopper feed was not adequately guarded to prevent employee access. b) On or about March 13, 2025, at the fry?s kitchen, the interlocked guard on the extruder and cutter line lift gate did not adequately protect employees from residual movement of the machine. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) On or about, February 17, 2025, at the corn-chip department, the employer did not ensure that each operator was trained as a lockout authorized employee to control hazardous electrical and mechanical energies prior to engaging in service and maintenance activities. Operators did not know to shut down equipment before reaching into the unguarded hopper feed with their hands to dislodge stuck material during jams. Operators were exposed to caught-in and amputation hazards from the rotating auger screw at the lower section of the hopper. b) On or about March 13, 2025, at the fry's kitchen, the employer did not ensure that each operator was trained as a lockout authorized employee to control hazardous electrical stored energy prior to engaging in service and maintenance activities. Operators did not know to shut down equipment before reaching through a lift gate to remove material during jams. Operators were exposed to unexpected cutter movement likely to result in finger amputation. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
