29 CFR 1926.501(a)(2): The employer shall determine if the walking/working surfaces on which its employees are to work have the strength and structural integrity to support employees safely. Employees shall be allowed to work on those surfaces only when the surfaces have the requisite strength and structural integrity. On or about February 27, 2025, the roof of the quality lab was not determined to have the strength and structural integrity to support an employee while they were building a ceiling extension. The employee was on the roof of the quality lab to finish the extension project and fell through the roof.

29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. On or about February 27, 2025, an employee was not protected from falling approximately 8 ft 5 inches to equipment through a hole in the roof of the quality lab. The employee was attempting to finish the ceiling extension project and put a permanent cover over the hole.
