The rotating drill chuck was not guarded, employees were not provided protection from flying chips and debris.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: Employees engaged in grinding activities in Building #2 at the Victor Bench Grinder Serial G16737A were exposed to struck-by hazards in that the work rest opening were not maintained at a maximum of one-eight inch. The work rest was found to be approximately three-fourths and one-half inches away from the grinding wheel.
