(a) Machine area - On or about March 5, 2025 and at times prior thereto, the employees were exposed to electrical hazards from contact with live electrical conductors through the unused openings on the A-B Quality Start Stop switch for the Reefer Space Fan.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): Freezer Hold: On or about March 5, 2025, and at times prior thereto, a flexible cord for a JBL speaker was not visually inspected prior to use. The black cord was observed with a damaged plastic connector which had copper wire exposed. Employee(s) were potentially exposed to electrical shock hazard(s).
