29 CFR  1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.   a) On or about March 06, 2025, the employer did not have a complete energy control program that included specific energy isolation procedures, training for authorized, affected and other employees and periodic inspections of the procedures on an annual basis for equipment such as, but not limited to; the HYD-Mech bandsaw H28A-120.

29 CFR  1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  (2) turn off and shut down the machine (3) physically locate and operate the energy isolating devices in such a manner as to isolate the machine or equipment from the energy sources.  (4) affix lockout or tagout devices to each energy isolating device (5) following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe.  (6) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergizing of the machine or equipment have been accomplished.    a) On or about March 06, 2025,  the employer did not ensure that in the Middle Saw Bay-2 Pod area employees controlled the electrical and hydraulic hazardous energy of Hyd-Mech H28A bandsaw utilizing sequence (d)(1) through (d)(6) while changing blades, exposing employees to the point of operation of the saw.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.     c) On or about March 06, 2025, in the Middle Saw Bay 2 area, the employer did not ensure that the Hyd-Mech H28A bandsaw auger was guarded to protect employees from getting caught in the rotating auger.

29 CFR  1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.  a) On or about March 06, 2025 the employer did not ensure that in the Middle Saw Bay 2 location by the conveyor, the chains and sprockets under the conveyor were completely guarded exposing employees to caught in hazards.
