29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. On or about March 4, 2025, and at times prior thereto, employees worked from the platform of an elevated Mallaghan Aerial Device without the walking-working surface being free of loose mats while troubleshooting an aircraft auxiliary power unit.

29 CFR 1910.28(b)(15): Except as provided elsewhere in this section or by other subparts of this part, the employer did not ensure that each employee on a walking-working surface 4 feet (1.2 m) or more above a lower level is protected from falling by one of the systems listed in 1910.28(b)(15)(i)-(iii). On or about March 4, 2025, employees were working from the elevated platform of a Mallaghan Aerial Device without the use of a fall protection system.
