29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. On or about March 7, 2025, at the above-mentioned workplace, the written energy control program did not include specific step-by-step procedures to control and lockout energy sources for employees who may perform service and/or maintenance on all multi-source energy equipment such as, but not limited to, press brakes, presses, turret punch presses, and air compressors. Employees were thereby exposed to the hazards associated with unexpected energization of equipment.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device prior to working on the machine or equipment. a. On or about March 7, 2025, at the above-mentioned workplace, personal lockout or tagout devices were not attached to a group lockout device when employees perform service and/or maintenance, to include preventative maintenance, on all multi-source equipment, such as, but not limited to press brakes, presses, turret punch presses, and air compressors. Employees were thereby exposed to hazards of unexpected re-energization of equipment.
