On or about March 5, 2025, on the upper level of a residential building, an employee was not in use of fall protection while performing framing work activities from a work level that was more than six(6) feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about March 5, 2025, on the east side of the building; employees accessed the roof using an extendable ladder and the siderails did not extend at least three(3) feet above the landing.
