Site: On or about March 10, 2025, and times prior to, the employer did not ensure that the Rademaker croissant machine was locked out and tagged out prior to an employee beginning the scheduled cleaning of the machine. The employee sustained lacerations and crushing injuries to her fingers when her hand became entrapped between moving rollers.
