On or about December 16, 2024, in the facility, a forklift operator was involved in an accident resulting in damage to company property. The employer failed to ensure that prior to allowing an employee to operate a powered industrial vehicle after being involved in an accident, the affected employee was provided with refresher training and re-evaluation.
