29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At the jobsite located at 1041 S.127th Street, Wichita KS, 67207 employees were exposed to fall hazards of approximately 12 feet 11 inches in that the 17 foot Werner adjustable Ladder and 17 foot Little Giant adjustable ladder were not properly adjusted to the required 3' height above the landing surface.
