29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. a. On or about April 5, 2025, an employee in the Compounding Department did not receive effective information and training on the exothermic hazards associated with sodium hydroxide when added to hot water. As a result, the employee sustained burns to his face, head and neck when he added an approximately 50-lb bag of sodium hydroxide pellets into an approximately 55-gallon drum of hot water causing a chemical reaction. b. On or about May 23, 2025, an employee in the Compounding Department did not receive effective information and training on the exothermic hazards associated with sodium hydroxide when added to hot water. As a result, the employee sustained burns when he added an approximately 50-lb bag of sodium hydroxide pellets to an approximately 55-gallon drum of hot water causing a chemical reaction. c. c. On or about June 10, 2025, an employee handling Jaguar C-17 and Jaguar C-162, proprietary guar hydroxypropyltrimonium chloride products, did not receive effecting information and training on the potential health and physical hazards of the chemical, a respiratory irritant and potential combustible dust.

Employee training shall include at least: 29 CFR 1910.1200(h)(3)(ii): The physical, health, simple asphyxiation, combustible dust, and pyrophoric gas hazards, as well as hazards not otherwise classified, of the chemicals in the work area; a. On or about April 5, 2025, employees did not receive training on the exothermic properties of sodium hydroxide, which exposed them to burn hazards. b. On or about June 10, 2025, employees did not receive training on the combustible dust hazards associated with guar hydroxypropyltrimonium chloride, which exposed them to conflagration hazards.
