29 CFR 1910.133(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. a. On or about April 5, 2025, in the compounding department, the employer did not ensure that employees utilized safety goggles and appropriate face protection while performing batch mixing activities involving sodium hydroxide, thereby causing chemical burns to the employee's face. b. On or about May 23, 2025, in the compounding department, the employer did not ensure that employees utilized appropriate face protection while performing batch mixing activities involving sodium hydroxide, thereby causing chemical burns to the employee's face. c. On or about May 28, 2025, in the compounding department, the employer did not ensure that employees utilized safety goggles and appropriate face protection while performing batch mixing activities involving sodium hydroxide, thereby exposing employees to thermal and chemical hazards.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a. On or about April 5, 2025, an employee was hospitalized due to a work-related chemical splash incident involving sodium hydroxide, the employer failed to report the in-patient hospitalization to OSHA within twenty-four hours.
