29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: A) Elite Concrete NY LLC does not ensure that subcontractors and or other workers working in proximity to energized power lines are protected against electric shock by de-energizing and grounding, effectively guarding, insulating or other means. Between the dates of 12/2/24 through 3/11/25, at 1 Mallory Ave., Jersey City NJ -Slab -Parallel to Mallory Ave., (a): Installation of Concrete Slab - where concrete subcontractor Employee A was exposed to electrocution and electric shock hazards while engaged in the finish of the third floor slab utilizing an alumnimun trowel on a multi-story residential building approximately 6 feet from a power line that was not de-energized and grounded, guarded, insulated or protected by other means.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement: a)At or near 1 Mallory Ave., Jersey City NJ - Perimeter of Slab - Ground Level: Employees and other workers were exposed to empalement hazards while engaged in various activities over approximately 3 feet long rebar pieces, including but not limited to installation of forms, rebar and concrete, for a period of 2/14/25 to 3/3/25. b)At or near 1 Mallory Ave., Jersey City NJ - Perimeter of Slab - Ground Level: Employees and other workers were exposed to empalement hazards while engaged in various activities over approximately 3 feet long rebar pieces, including but not limited to removal of concrete blankets and adjustble steel shoring poles, on or about 3/11/25.
