29 CFR 1910.28(b)(3)(iv): Each employee is protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used; a) On or about March 18, 2025, at a workplace located at 200 West Front Street, Peshtigo, Wisconsin in the Beater Room; employees accessing the elevated platform at the end of the "Big Green" conveyor were not protected from a fall hazard of 5 feet as the ladder access was not equipped with a self closing swing gate. b) On or about March 18, 2025, at a workplace located at 200 West Front Street, Peshtigo, Wisconsin in the Beater Room; employees accessing the elevated platform adjacent to the air exchange unit were not protected from a fall hazard of over 15 feet as the ladder access was not equipped with a self closing swing gate.

The switch was hanging on the tote stand with a zip tie. b) On or about March 18, 2025, at a workplace located at 200 West Front Street, Peshtigo, Wisconsin in the Retention Aid Polymer Room; a 120V ground fault circuit interrupter outlet was not securely mounted to a fixed structure.
