On or about March 18, 2025, in the Mechanically Separated Chicken (MSC) area, employees perform nightly cleaning/sanitation tasks on augers and moving parts on machines such as dual screw conveyor, mechanical separator, and finish meat pump machines and the procedures do not specify how to disconnect the electrical energy source, where to place the lockout device(s), and did not specify how to release the machines stored or residual energy, among other deficiencies.

On or about March 18, 2025, in the Mechanically Separated Chicken (MSC) area, employees perform nightly cleaning/sanitation tasks on augers and moving parts on machines such as dual screw conveyor, mechanical separator, and finish meat pump machines and the employer does not ensure that all steps required for the control of hazardous energy sources as outlined in 29 CFR 1910.147(d), is performed, resulting in employees not de-energizing and locking out all sources of energy for the  FTS Flamboro Dual Screw Conveyor and interconnected machines.
