29 CFR 1910.23(b)(9): The employer did not ensure that ladders  are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury.  On or about March  25, 2025, an employee was engaged in commercial maintenance activities, located at 9811 North 135th Street, Waverly, NE 68462 was exposed to fall hazards in that the employee was accessing and egressing an unprotected mezzanine located 12-feet 10 and ?-inches above the lower walking working surface with a  ladder that had not been inspected.  The employers failure to ensure that ladders are inspected and removed from service when defects are found led to a ladder sliding due to excessive wear on the feet causing an employee injury.
