At the worksite, located at 1101 W Pipkin Rd Lakeland, Fl 33811; On or about 3/12/25, the employer exposed employees to struck by hazards in that eye protection was not provided for employees using pneumatic nail guns and circular saws while conducting framing and sheathing operations on the roof of a commercial building.

29 CFR  1926.452(k)(4):Ladders used to support ladder jacks were not placed, fastened, or equipped with devices to prevent slipping:  a.) At the worksite, located at 1101 W Pipkin Rd Lakeland, Fl 33811; On or about 3/12/25, the employer exposed employees to fall hazards of approximately 10' in that a ladder jack scaffold was not properly placed due to the ladder foot pads were not inserted into the ground to prevent the scaffold from slipping or shifting on the uneven surface.
