(a)	Located at 3321 Toy Road, Groveport, Ohio, 43125: On or about March 25, 2025 and times prior, the employer failed to ensure that the storage racks located at or near 11-23-01-B, 03-55-02-A, 02-51-01-B, and 02-43-01-A were free from column deflection damage ranging from 1/2 inch to approximately 2 inches.  (b)	Located at 3321 Toy Road, Groveport, Ohio, 43125: On or about March 25, 2025 and times prior, the employer failed to ensure that the storage rack located at or near 13-24-02-B was free from damage were the row spacer had been broken and bent which caused it to detach from the bolt connecting it to the racking system.   (c)	Located at 3321 Toy Road, Groveport, Ohio, 43125: On or about March 25, 2025 and times prior, the employer failed to ensure that the storage rack located at or near 14-27-01-A was free of twisted footplates of approximately 1 inch.  (d)	Located at 3321 Toy Road, Groveport, Ohio, 43125: On or about March 25, 2025 and times prior, the employer failed to ensure that the storage racks located at or near 14-28-02-A,  A, and 07-56-01-A were free from damage from broken welds on the strut supports of the racking system.
