"OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: On March 31, 2025, and at times prior thereto, at AZZ Galvanizing - St. Louis located at 1461 Kin Ark Ct, Saint Louis, MO, 63132: Employees were exposed to struck-by and crushing hazards while loading and strapping metal H-frame trusses onto a flatbed trailer for transport. Employees worked within the fall shadow of trusses that were loaded vertically, on their narrow edge, and unstable.

29 CFR 1910.22(d)(2): The employer did not ensure that hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again. The employer did not ensure that if the correction or repair cannot be made immediately, the hazard is guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired. At 1461 Kin Ark Ct, Saint Louis, MO, 63132, On March 31, 2025 and at times prior thereto, employees were exposed to struck by and crushing hazards in the loading yard area. The loading yard area was discovered to have multiple large water-filled holes, which made the walking-working surface uneven. Employees, vehicles, and powered industrial truck traffic with loads were traversing through the area. The loading area was not guarded to prevent employees from using the walking-working surface until the holes could be repaired.
