29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  At a new residential worksite located at 2351 NW Palisades Dr, Riverside MO -- An employee working from an extension ladder directly below an employee working from a job-made scaffold was not wearing any head protection. Another employee working below a different employee who was framing a deck above, was not wearing head protection. These hazards exposed the employees working below to struck-by hazards from tools and/or materials. This was documented on or about 03/10/2025.

The employer exposed the employees to these hazards by allowing them to work from a scaffold attached to the rear of the structure that was not designed by a qualified person.
