1910.132(f)(3)(iii): The employer did not retrain employees when inadequacies in the affected employee's knowledge or use of assigned PPE indicated the employee had not retained the requisite understanding or skill: a) On or about March 30, 2025, in the production area, the employer failed to provide retraining on personal protective equipment (PPE) to employees required to replace the filters on the Deordizer equipment. Employees were, thereby, exposed to thermal and pressure energy hazards.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. On or about March 30, 2025, employees were exposed to equipment hazards associated with Deordizer pressurized fluid. The employer failed to implement energy control application steps as the equipment was not shut down or turn off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving equipment isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from servicing hazards.
