29 CFR 1926.20(b)(2):  The employer did not provide for a program of frequent and regular inspections of the job sites, materials, and equipment to be made by competent person:   a) Job site 904 Jackman Road, Avalon, PA: On or about April 4, 2025 during roofing activities, the employer did not ensure a system was in place to identify and correct unsafe working conditions. Deficiencies noted during the inspection included, but were not limited to, not using fall protection where fall hazards of up to at least 20 feet existed, not blanketing/insulating an electrical service drop and not ensuring that the ladder used to get up/down to and from the roof was properly extended above the roof line.

29 CFR  1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:   a) Job site 904 Jackman Road, Avalon, PA: On or about April 4, 2025 during roofing activities, the employer did not ensure the ladder used to get up/down to and from the roof was properly extended above the landing. It was determined that it was only extended approximately 32 inches above the roofline.
