29 CFR 1910.147(c)(7)(iii)(C). The retraining shall reestablish employee proficiency and introduce new or revised control methods and procedures, as necessary. (a) Facility: On or about April 4, 2025, employees were working on a 556 gallon atmospheric tank when the tank failed due to a build up of pressure inside the tank. The company did not develop, document nor utilize any lockout/tagout procedures for this tank. This tank was used to hold condensate overflow from the boiler/manufacturing systems.
