29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  On or about April 8, 2025,  the employer Quinetero Roofing LLC at jobsite located at 1184 Dover Center Road, Bay Village, OH, did not ensure that each affected employee used appropriate eye or face protection when preforming roofing work on an existing residential structure while exposed to potential struck by hazards (flying particles/debris) while using pneumatic nail guns.

29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about April 8, 2025, employees were exposed to a struck by hazard while working 9.8 feet beneath an active roofing crew at a residential home located at 441 Cherry Ridge Road, TALLMADGE, OH 44278, without wearing protective helmets.  QUINTERO ROOFING LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR  1926.100(a) , which was contained in OSHA inspection number 1785938, citation number 1, item number 1 and was affirmed as a final order on December 18, 2024, with respect to a workplace located at 441 Cherry Ridge Road, Tallmadge, OH.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about April 8, 2025, the employer did not ensure employees engaged in preparing a roof for shingle installation with an 8/12 pitch at a residential home located at 1184 Dover Center Road, Bay Village, OH, 9 feet above the ground were protected by guardrail systems, safety net system, or personal fall arrest system.   Quintero Roofing, LLC was previously cited for a violation of this occupational safety and health standard, 1926.501(b)(13), which was contained in OSHA inspection number 1785938, citation number 2, item number 1 and was affirmed as a final order on 12/18/2024 , with respect to a workplace located at 441 Cherry Ridge Road, Tallmadge, OH.   Quintero Roofing, LLC was previously cited for a violation of this equivalent occupational safety and health standard,  1926.501(b)(11), which was contained in OSHA inspection number 1601617, citation number 1, item number 1 and was affirmed as a final order on 08/17/2022, with respect to a workplace located at 1041 East Turkeyfoot Lake Rd Akron, Ohio 44312.
