29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. (a) On or about April 9, 2025, at the jobsite where employees were using the scaffold frame as means of access and egress from the working level of the scaffold.

29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system. a) On or about April 9, 2025, on the south side of the jobsite, where employees were applying bricks to the exterior of the building from the second tier of a four tier scaffold without any fall protection system.
