29 CFR 1926.1053(b)(13): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) On April 10, 2025, at the above addressed jobsite, the portable ladder used by employees to access and egress the roof of the house, measuring approximately 24 feet from ground to eve, did not have the side rails extend at least three (3) feet above the upper landing. No abatement certification or documentation is required for this item.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were the employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) On or about April 10, 2025, at the above-mentioned address, the employees were exposed to fall hazards of approximately 24 feet ground to eave while performing residential roofing work. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
