29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. (a) On or about April 14, 2025, the employer did not ensure that the nailing heads of the Viking Champion Automated Pallet Assembler 304A were guarded to prevent the operator's access during pallet assembly. A machine operator was placing boards on the pallet's stingers and then holding them down with their fingers and hands exposed to caught-in between hazards likely to result in serious injuries due to vertically moving pneumatic nailing heads driving nails into lumber pieces to assemble pallets. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. (a) On or about April 14, 2025, the employer did not ensure that a pinch point created by the moving chain and rotating sprocket wheel was guarded to prevent inadvertent access at the infeed side of Viking Champion Automated Pallet Assembler 304A. Employees working in that area were exposed to caught-in hazards likely to result in serious injury when the chain and sprocket were indexing to move the flipping arms. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and cables may not be used as a substitute for the fixed wiring of a structure; (a) On or about April 14, 2025, an orange extension cord was used to power a PW90 water dispenser instead of an outlet (a local branch circuit receptacle). The cord was zip-tied to a horizontal conduit and ran along the wall between the Viking Pallet Assembler and nearby work stations to connect to a three-way adapter plug. Employees were exposed to electrical hazards associated with unsafe use of an extension cord to power a water dispenser. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
