29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards.  This was most recently documented on or about April 8, 2025 when roofers were observed installing shingles and working at height.  Roofers were exposed to a fall distance of 11 feet eave to ground without the use of fall protection.  The Gustavo Rodriguez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1784451, citation number 1, item number 2, and was affirmed as a final order on 12-12-2024, with respect to a workplace located at 3314 North Bedford Ct. (Lot #41) Wichita, KS.  The Gustavo Rodriguez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1654487, citation number 1, item number 1, and was affirmed as a final order on 06-14-2023, with respect to a workplace located at 2315 East 159th Cir. 29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access:  The employer is failing to protect employees from fall hazards.  This was most recently documented on or about April 8, 2025 when roofers were observed installing shingles and working at height.  Roofers were exposed to a fall distance of 11 feet eave to ground in that the Aluminum ladder used to access the roof was not extended at least 3 feet above the eave and was not secured to the structure.  The Gustavo Rodriguez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1784451, citation number 1, item number 3, and was affirmed as a final order on 12-12-2024, with respect to a workplace located at 3314 North Bedford Ct.
