29 CFR 1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   a) New Construction located at 6803 Spout Springs Road, Flowery Branch, GA 30542, Rear external wall: On or about 7/2/2025, the employer exposed employees to struck-by hazards while operating pneumatic nail guns, in that eye protection was not provided to employees engaged in siding installation activities operating these tools at 120 PSI.

29 CFR  1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:   a) New Construction located at 6803 Spout Springs Road, Flowery Branch, GA 30542, Front side scaffold: On or about 7/2/2025, the employer exposed employees to fall hazards of 15-feet to the lower ground level, in that a fall protection system was not provided while using a ladder jack scaffold to install siding.
