Cedar Tree Street Park City, KS 67219, employees engaged in various ground hand activities were exposed to burn hazards and electrical shock in that the cord of the DeWalt saw was damaged and wrapped with black electrical tape. Employees were observed using the unsafe condition tool to saw the wood decking.

The employer did not ensure that the pink, light green, and orange flexible extension cords, used in roof decking installations, were free from recognized hazards in that they had multiple areas of damaged, frayed and twisted insulation. Additionally, the cord of the DeWalt saw was damaged and wrapped with electrical black tape.

29 CFR 1910.334(a)(2)(i): Portable cord- and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence for possible internal damage (such as pinched or crushed outer jacket):  At the jobsite located at 2121 E. Cedar Tree Street Park City, KS 67219, employees engaged in various ground hand activities were exposed to burn hazards and electrical shock. The employer did not conduct any visual inspection of portable cord and plug equipment, and flexible cord sets before use on any shift for external defects. The pink, light green, and orange flexible extension cords, used in roof decking installations were observed with multiple areas of damage to outer jacket, frayed and twisted insulation. Additionally, the cord of the DeWalt saw was damaged and wrapped with black tape.  The extension cords were being used to energize the DeWalt saws.

Cedar Tree Street Park City, KS 67219, employees engaged in various ground hand activities were exposed to burn hazards and electrical shock. The employer permitted employees to use the DeWalt saw that had a damaged to the power supply cord, which was not removed from service, after identifying damage to the cord and wrapped it with electrical tape to cover the inner insulated wire conductors. The pink, light green, and orange flexible extension cords, used in roof decking installations were observed with multiple areas of damage to outer jacket, frayed and twisted insulation, and were not removed from service. The extension cords were being used to energize the DeWalt saws.
