29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a.) Steel Valley Roofing & Sheet Metal, Inc. 3009 Wilmington Rd, New Castle, PA 16105, Ridgewood Room roof valley: On or about 04/11/2025 and at times prior thereto, Employees were exposed to fall hazards while walking across the Ridgewood Room roof valley without the use of a guardrail systems, safety net systems, or personal fall arrest systems. Employees were exposed to fall hazards of 8 feet.   b.) Steel Valley Roofing & Sheet Metal, Inc. 3009 Wilmington Rd, New Castle, PA 16105, Ridgewood Room steep Roof: On or about 04/11/2025 and at times prior thereto, Employees were exposed to fall hazards while walking across the Ridgewood Room roof without fall protection. Employees were exposed to fall hazards of 21-feet with approximately 6:12 roof pitch.  c.) Steel Valley Roofing & Sheet Metal, Inc. 3009 Wilmington Rd, New Castle, PA 16105, Nellcom Technologies flat roof area: On or about 04/11/2025 and at times prior thereto, Employees were exposed to fall hazards while walking across the Nellcom Technologies flat roof without the use of a guardrail systems, safety net systems, or personal fall arrest systems. Employees were exposed to fall hazards of 9 feet to the ground level.  d.) Steel Valley Roofing & Sheet Metal, Inc. 3009 Wilmington Rd, New Castle, PA 16105, Nellcom Technologies steep roof: On or about 04/11/2025 and at times prior thereto, Employees were exposed to fall hazards while walking across the Nellcom Technologies steep roof without the use of a guardrail systems, safety net systems, or personal fall arrest systems. Employees were exposed to fall hazards of 9 feet with approximately 7:12 roof pitch.

Steel Valley Roofing & Sheet Metal, Inc. 3009 Wilmington Rd, New Castle, PA 16105, Crane Room Grille roof: On or about 04/11/2025 and at times prior thereto, Employees were exposed to fall hazards while working around the unprotected skylights on the flat Crane Room roof. Employees were exposed to fall hazards of 31-feet.

Steel Valley Roofing & Sheet Metal, Inc. 3009 Wilmington Rd, New Castle, PA 16105, Crane Room Grille roof: On or about 04/11/2025 and at times prior thereto, employees were performing roof work operations with an incomplete warning line. Employees were exposed to fall hazards of 30 feet. The West and North side of the roof did not have a complete warning line.

Steel Valley Roofing & Sheet Metal, Inc. 3009 Wilmington Rd, New Castle, PA 16105, Crane Room Grille roof: On or about 04/11/2025 and at times prior thereto,  employees were performing roof work operations on the Crane Room Grille roof. The South side warning line was 2 feet from the edge of the roof and less than 4 feet from the edge of the roof on the North side.

Steel Valley Roofing & Sheet Metal, Inc. 3009 Wilmington Rd, New Castle, PA 16105, Ridgewood Room Roof: On or about 04/11/2025 and at times prior thereto, A roof assessment and/or inspection was not completed before employees began walking on the Ridgewood Room roof to determine its strength and structural integrity to support them while on the roof.  An employee fell through the roof and landed in the interior of the building.
