29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) Worksite - On or about April 15, 2025, a worker was operating a JLG 10054 telehandler with a large wooden box attached to the front forks. The wooden box is not an approved manufacture attachment for the JLG exposing employees to struck-by hazards.
