29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: On or about April 15, 2025, at an establishment located at 232 Bosworth Lane in Neenah, Wisconsin, employees of We Do Roofing, LLC were performing residential roofing activities and were not utilizing a designed anchor point capable of supporting 5000lbs while exposed to fall hazards of over 18 feet.

29 CFR 1926.502(d)(17): The attachment point of the body belt shall be located in the center of the wearer's back. The attachment point of the body harness shall be located in the center of the wearer's back near shoulder level, or above the wearer's head. On or about April 15, 2025, at an establishment located at 232 Bosworth Lane in Neenah, Wisconsin, employees of We Do Roofing, LLC were performing residential roofing activities with a fall hazard greater than six feet and did not attach their fall arrest components to a point near the center of their back.

29 CFR 1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502. Note: There is a presumption that it is feasible and will not create a greater hazard to implement at least one of the above-listed fall protection systems. Accordingly, the employer has the burden of establishing that it is appropriate to implement a fall protection plan which complies with 1926.502(k) for a particular workplace situation, in lieu of implementing any of those systems. On or about April 15, 2025, at an establishment located at 232 Bosworth Lane in Neenah, Wisconsin, employees of We Do Roofing, LLC were performing residential roofing activities with no form of conventional fall protection while exposed to fall hazards of over 9 feet. We Do Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1675659, citation number 1, item number 1 and was affirmed as a final order on July 24, 2023, with respect to a workplace located at 2762 Kadlec Drive in Beloit, WI 53511. We Do Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1591715, citation number 1, item number 2 and was affirmed as a final order on September 26, 2022, with respect to a workplace located at 3924 Woodland Road, Howard, WI 54313. We Do Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1587305, citation number 1, item number 1 and was affirmed as a final order on July 29, 2022, with respect to a workplace located at 223 Air Park Drive, Watertown, WI 53094.
