29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On or about April 15, 2025, the employer did not ensure that employees implemented energy control steps prior to performing servicing and maintenance activities on the #1 thermoforming line. Hazardous energy was not isolated prior to employees troubleshooting and setting up the equipment, exposing them to struck-by and caught-between hazards from moving parts in the event of unexpected start-up or movement of the machine. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards. b) On or about April 15, 2025, the employer did not ensure that employees implemented energy control steps prior to performing servicing and maintenance activities on the B1 Stacker. Hazardous energy was not isolated prior to employees troubleshooting and setting up the equipment, exposing them to struck-by hazard and caught-between hazards from moving parts in the event of unexpected start-up or movement of the machine. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about April 15, 2025, the employer did not ensure that the forming dies on the thermoforming line #3 were guarded. The side gate guard was opened while machine was in operation, exposing employees to crushing injuries from moving parts while the machine cycled. b) On or about April 15, 2025, the employer didn't ensure that the operation station on the stacking area and the end of thermoforming line #3 was guarding to protect employees from crushing injuries from moving parts while the machine cycled. c) On or about April 15, 2025, the employer didn't ensure that the operation station on the stacking area and the end of thermoforming line #5 was guarding to protect employees from crushing injuries from moving parts while the machine cycled. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
