This violation was most recently observed at or near 1522-1532 W Girard Ave, Philadelphia PA on or about April 16, 2025, where employees were climbing the cross braces to access the scaffold to do caulking around the windows and exposed to a fall hazard of approximately thirty five (35) feet in height to the ground level below.

This violation was most recently observed at or near 1522-1532 W Girard Ave, Philadelphia PA, Right Exterior Scaffold, on or about April 16, 2025, where employees were working on a scaffold without the use of toeboards, thereby exposing employees to struck-by hazards from items such as but not limited to buckets of material.

This violation was most recently observed at or near 1522-1532 W Girard Ave in Philadelphia, Pennsylvania, Left Exterior Scaffold, on or about April 16, 2025, where employees were conducting window caulking activities on a scaffold that lacked top rails and mid-rails exposing employees to fall hazards of approximately thirty five (35) feet in height to the ground level below without the training to recognize the hazards.   b) The employer does not have each employee who performs work on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.
