(a) Worksite - Rooftop: On or about 4/17/2025, the employer did not ensure that rooftop work areas and passageways were kept clear of building materials, work tools, and debris creating slip, trip, and fall hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a) Worksite: On or about 4/17/2025, an employee was exposed to fall hazards while performing rigging and crane communication work approximately 110 feet above the ground floor without fall protection.

29CFR 1904.41(a)(1)(i): If your establishment had 20-249 employees at any time during the previous calendar year, and your establishment is classified in an industry listed in appendix A to subpart E of this part, then you must electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form.  (a) On or about 4/17/2025, the employer failed to electronically submit information from their OSHA Form 300A or equivalent by 03/02/2025.
