On or about April 17, 2025, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries. DBA Westside Contractors Inc. was previously cited for a violation of this occupational safety and health standard 1926.102(a)(1), which was contained in OSHA inspection number 1728692, citation number 1, item number 1 and was affirmed as a final order on May 17, 2024, with respect to a workplace located at 1494 Woodscreek Cir., Crystal Lake, IL 60014.

On or about April 17, 2025, at the above addressed jobsite, an employee performing residential roofing operations, approximately twelve (12) feet three (3) inches above the ground, was not protected from falls, thereby exposing employee to fall hazards.  b.  On or about April 17, 2025, at the above addressed jobsite, an employee performing residential roofing operations, approximately eight (8) feet eight (8) inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  c.  On or about April 17, 2025 at the above addressed jobsite, employees performing residential roofing operations, approximately eight (8) feet four (4) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.
