29 CFR 1926.20(b)(2): A program was not in place that provided for frequent and regular inspections of the job sites, materials, and equipment to be made by a competent person: a) Job site, 125 North Craig Street, Pittsburgh, PA: On or about April 15, 2025 during construction activities, the employer did not ensure a system of hazard recognition and abatement was implemented. Deficiencies noted during the inspection included, but were not limited to, not ensuring railings along the auditorium balcony were of appropriate height to serve as fall protection, not having fire extinguishers in locations where they were in way of traffic and/or not being mounted with signage and not having a top rail along window opening.
