29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. On or about April 17, 2025, at an establishment located at 7801 Stuart Avenue, Wausau, Wisconsin, in the Coating Department, the employer did not conduct a periodic inspection of their energy control procedures to ensure that the procedures and the requirements of this standard were followed. Employees performed servicing and maintenance tasks on equipment including but not limited to, Coater Line 9 Head 1 when the last periodic inspection had been performed in June of 2023.

On or about April 17, 2025, at an establishment located at 7801 Stuart Avenue, Wausau, Wisconsin, in the Coating Department on Coater Line 9, an employee engaged in equipment changeover task on Head 1, was not adequately trained in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control as an authorized employee.

29 CFR 1910.147(f)(1): Testing or positioning of machines, equipment or components thereof. In situations in which lockout or tagout devices must be temporarily removed from the energy isolating device and the machine or equipment energized to test or position the machine, equipment or component thereof, the sequence of actions identified in 1910.174(f)(1)(i)-(v). On or about April 17, 2025, at an establishment located at 7801 Stuart Avenue, Wausau, Wisconsin, in the Coating Department on Coater Line 9, the employer did not ensure an employee conducting testing and position of Head 1 was protected from machine hazards while ensuring the silicone product was tracking correctly on rotating rollers. An employee entered the hazard area to wipe flaking chips off the rollers and was exposed to a caught-in hazard.
