29 CFR 1910.243(c)(1): Abrasive wheels were used on portable grinders which were not provided with safety guards meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4)(a). On or about April 24, 2025, the employer failed to implement guards for hand grinders used for grinding operations on the Welding Area shop floor. Operations include but are not limited to grinding down various types of steel including carbon steel, exposing employees to struck by and burn hazards.

29 CFR 1910.132(d)(2): The employer failed to verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. On or about April 24, 2025, the employer failed to create and implement a written Personal Protective Equipment (PPE) hazard assessment for employees conducting welding and grinding operations on the shop floor. Operations include but are not limited to grinding down and repair welding various types of steel including carbon steel, exposing employees to struck by and burn hazards.
