On or about April 24, 2025, at the above addressed jobsite, a damaged wire rope sling used to lift truss panels was not inspected by a competent person and removed from service, thereby exposing employees to struck-by hazards.

1926.503(b)(1)  The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.  a. On or about April 24, 2025, at the above addressed jobsite, employees were exposed to hazards associated with unsafe powered industrial vehicle operations while lifting roof trusses by the direct placement of rigging equipment (i.e., wire rope sling) onto the tine of a  TEREX TH0606 A Telehandler for a below-the-tines lift, which could affect the capacity and/or safe operation of the equipment.  The employer did not receive written approval from the manufacturer of the equipment before using the sling.

The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  a. On or about April 24, 2025, at the above addressed jobsite, the employer failed to provide certification that each operator required to operate the TEREX TH0606A Telehandler has been trained and evaluated.   Mead Contracting, LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1910.178(l)(1)(i), which was contained in OSHA inspection number 1761355, citation number 1, item number 1, and was affirmed as a final order on December 9, 2024, with respect to a workplace located at 2155 Runway Dr.

On or about April 24, 2025, at the above addressed jobsite, an employee performing residential framing carpentry operations, eleven (11) feet three (3) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.  b.  On or about April 24, 2025, at the above addressed jobsite, an employee performing residential framing carpentry operations, eleven (11) feet two (2) inches above ground, was not protected from falls, thereby exposing the employee to fall hazards.  Mead Contracting, LLC was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1741383, citation number 1, item number 1, and was affirmed as a final order on June 11, 2024, with respect to a workplace located at 2348 Runway Dr. in New Lenox, Illinois.
