1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: a) On or about April 25, 2025, at the above addressed jobsite, the daily inspections by the competent person prior to the start of the work and as needed throughout the shift did not fully confirm whether employee exposure to a hazard could be reasonably anticipated while performing certain work: (1) utilizing a split portion of an extension ladder or backfill for egress from the trench excavation. (2) whether there were loose rocks present that could fall into the trench. (3) whether backfilling of the completed excavation work adjacent to the trench box should be conducted. (4) whether and when a rigging chain should be removed from service. (5) whether the manufacturer?s written tabulated data for the steel shield system were available at the site. (6) whether there was damage or a defect in a lower spreader on a trench box that might impair its proper function.
