(a) On or about April 24, 2025, at N15W29589 Brookstone Circle, Pewaukee, WI, the employer did not ensure employees conducting framing operations, using hand tools and nail guns were wearing eye protection. This exposed the employees to struck by hazards and eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about April 24, 2025, at N15W29589 Brookstone Circle, Pewaukee, WI 53072, employees installing sheathing on a new residential home with a 6:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 12 feet from the roof eve to the ground below on the back (south) side of the home.
