29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person: On or about April 23, 2025, during a residential roofing job at a job site located at 323 East Ave., Tallmadge OH 44278, the employer failed to perform inspections of the job site to ensure potential hazardous conditions were identified so that corrective action, such as but not limited to, the implementation and use of personal protective equipment such as fall protection was enforced.

Employees were working on the roof with no PFAS or other protections in place.
