29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented, and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about 04/28/2025, in the Kitchen Department, Lockout/tagout procedures were not utilized when changing the plates of the grinder machine, which exposed employees to injuries from rotating/pinch point hazards.   b) On or about 04/28/2025, in the Kitchen Department, Lockout/tagout procedures were not used when machine operators disassemble machines at the end of their shifts, including but not limited to the Grinder, the Extruder, and the Poly Clip, which expose employees to caught-in hazards. 29 CFR  1910.147(d)(1): Before an authorized or affected employee turns off a machine or equipment, the authorized employee shall have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy.  a) On or about 04/24/2025, in the Kitchen Department, the employee cleaning the extruder did not understand the type and magnitude of the energy, the hazards associated with that energy, and the proper methods for controlling it before turning the extruder off to clean the discharge. The employee did not lock out the extruder or wait for the residual energy to dissipate before placing their hands in the machine. This led to an amputation injury from the rotating auger.  ABATEMENT DOCUMENTATION REQUIRED

29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation  a) On or about 04/24/2025, in the Kitchen Department and throughout the establishment; employees were exposed to amputation hazards during servicing and/or maintenance activities such as, but not limited to; assembling, disassembling, and cleaning machines such as but not limited to, the Extruder, the Grinder, and the Poly Clip. Employees were not trained as authorized employees to recognize hazardous energy sources or to use the proper procedures to control those hazards. Employees are exposed to amputation injuries when placing their hands in machines without performing the proper energy control procedures.    ABATEMENT CERTIFICATION REQUIRED
